Bring these items with you to the Student Placement Center
  1. A current copy of all immunizations.
  2. A copy of any tuberculin skin tests performed to detect exposure to Tuberculosis and any chest x-rays.
  3. Health insurance card(s).
  4. Completed Early Childhood Screening forms.

Your health care provider can fax an updated list of immunizations to the Health Office at 651-632-3704.  Be sure that your child's name and date of birth (DOB) is clearly indicated on the record.
